I think that there is a bug in the thermal runaway detection.
I have a sb2040 over canbus, and I have not had any indication that there should be any hardware issues (loose wires, etc.)
Hotend is a rapido which has a good thermistor connection.

The thermal runaway starts at line 5024 of the attached log.
If you scroll up you will notice a shutdown proceeded by a single negative thermistor result.
I would suggest that ANY thermistor reading in the negative should trigger an immediate shutdown. I suspect there is a bug in the state machine at verify_heater.py, line 87.
My guess is that because the target temperature didn’t change, the detection block was never entered, but I can’t verify this after the fact.

From reading through the log it looks like the MCU shutdown was performed when the first negative reading occurred. Never the less, the printer DID NOT shutdown, and only avoided a fire when the power switch was shut off.
If that is the case, perhaps the PWM for the heater should first be set to zero before a full machine SCRAM to avoid this as a possibility?
It appears Klipper shut down appropriately, and nothing more can be done here. When Klipper enters a shutdown state it turns off all heaters. If the hotend was still heating after the shutdown, then it was a hardware problem.
If klipper was shutdown, why was it still logging to the log file for a few minutes?
Klipper went into shutdown state but the klippy.py
host process remained active and logging as intended.

That doesn’t sound like a hardware issue. That sounds like the klipper software running on the SB2040 failed to shut down as requested.

EDIT: Per @koconnor in the comment below, the log does indicate that the heater was commanded off and the appearance in the graph below that the heater power remained at full after the shutdown is just the mcu reporting its last request:
Do you know whether the heater in fact remained at full power and continued heating after the shutdown?
That is a misunderstanding of the log.
The log indicates that the host detected a heating error (Transition to shutdown state: Heater extruder not heating at expected rate
) and commanded all the MCUs to shutdown. The sb2040 successfully reported shutdown (MCU 'sb2040' shutdown: Command request
). The MCU code is designed to command all heaters off in the event of a shutdown request. After that point, both the host and mcus are in a “shutdown state” designed so that they no longer process further requests (at least until the state is explicitly cleared with a FIRMWARE_RESTART command).
The log continues to report pwm=1.00
because of a quirk in how log reporting is done. Since the heater code is no longer running, it is just reporting its last request - it can’t make new requests because the MCU and host are both in a “no new requests state”. The shutdown itself, however, does indicate that the sb2040 MCU commanded the heater off. If the heater did not turn off, it generally indicates that the hardware had malfunctioned and the software was no longer able to command it.

Typically when the printer enters a shutdown state I’m aware of it due to all the fans turning on (its loud.)
In this case, I had my back to the printer and noticed the smell of burning plastic (not that strong.)
I turned around to the Voron with a completely filled case of smoke, and a considerable amount of smoke being pushed out the nozzle.
I then flipped the wall switch to disconnect power.
I did not recall hearing the fans go on full like normal during a typical shutdown event, and I think I would have notice that and turned sooner rather than noticing the smell first.
I often print at various levels of ‘unattended’ relying on fire detectors in the room to provide a backup in the case of the unthinkable. In this case the smoke was contained in the case and did not trigger the alarm.
Needless to say, I would like to track this down so that I can begin to trust printing.
The only possible cause I can come up with is that the printer was recently homed at the start of a print, bringing the toolhead to the back. The thermistor connection could have come in contact with the cable z track leading to the noise in the signal that should have triggered the shutdown. I haven’t seen anything like that since.
I’m perplexed.

Hello Kevin. I have managed to reproduce the bug a second time. The printer tried to catch on fire and had another thermal runaway.
This time it occurred at print start, just after bed mesh.
The printer DID move to a paused state, but the toolhead continued to heat. There was no chance of a short or physical hardware failure.

Note that the mmu DOES NOT shut down.
I’m guessing that the canbus goes down and it never receives the shutdown message.
Is there a way to improve the logging for the SB2040 so that we can see if it ever gets the canbus message?
Also, is the SB2040 at the toolhead doing the PID loop? Or is that still being driven from my main octopus board?
Most important question:
Can’t the SB2040 running klipper see that the temp is out of range and shut itself down?

Without touching the printer, reboot restores the reading of the thermistor and rules out any issues caused by momentary movement (such as a wire tugged.)
I should mention I’m on canbus, so not much can get pulled. The thermistor wire is very short, and goes straight to the sb2040

Have you verified that the mosfet on the SB2040 is not failing? I also don’t understand why the printer would move to a paused state in a thermal runaway event. It should shut everything down immediately, but you’ve not posted a new log to evaluate so all we can do is guess what happened.

Mosfet was verified to be closed by checking pins p6 and vcc and determining they are not connected (circuit open.)
Board was pulled and examined under magnification for damage. Nothing apparent.
Thermistor connection was manipulated with the board only connected to power, no abnormal values were observed. Not even tiny noise.
I can only conclude that something in the mcu is telling the mosfet to stay on.

The question came up: Can we debug if the SB2040 is really shut down?
The answer is: YES!
I videoed the thermal runaway.
During the runaway event the led that controls the heater displays flashes (PWM) so indeed the MCU is sending a “please heat” signal to the heater in a cycle.
In addition, the animation of the leds on the stealthburner toolhead continues to animate (addressable color LED’s), another process that is controlled by Klipper running at the toolhead.
@koconnor would these be on another thread that would continue after the shutdown event occurs? Is this a false conclusion?

If the MCU is in shutdown state and that pwm=1.000 value is just a logging quirk, what are the changing temp= values? If the MCU is not running, how can it still report different temps then?
Also, why did, as SailMad reported, the fans not start when the MCUs entered shutdown state? Same as with the LED animation of his toolhead. This wouldn’t be the case without the mcu still running.
Also, think about it, the underlying hardware issue is apparently a sensor reporting false values. What other non-software related issue could cause the PWM not being turned off? A stuck MOSFET? At the same time as sensors reporting wrong values? Could be, but seems as likely as winning the lottery. Twice in a row, as Sailmed had the problem a 2nd time.
Or the MCU crashing with the PWMs staying on? Same, why would that happen in coincidence, twice in a row. very unlikely. And, even if that was the case, it should’ve been catched by the watchdog.
Only possible explanation is software bug.
